Playwright Leonard Melfi, a seminal author of the emerging Off-Broadway movement in the 1960s, died in October 2001 of congestive heart failure. In an unusual record-keeping lapse--either by Mount Sinai Hospital of New York or that city's morgue--Melfi's family did not learn about his death until February of this year. ...
