WHEN I first worked in hospitals there was a thing called stress ulceration that killed people, and in the intensive care unit they used to put antacids into patients' stomachs to prevent it.
Along came H2 blockers and the problem almost disappeared. When sulcralfate arrived people wrote papers about which was better. Then came the meta-analysis, that showed sulcralfate was superior. But some Canadians did a Very Large Randomised Trial which showed the opposite, and a VLRT beats a meta-analysis like a flush beats a straight. We changed again.
