EDITOR I read with dismay Professor Leon Piterman's guest editorial 'No place for fee-for-service in future health system' (Gut Feelings, 25 August) advocating abolishing fee per item in favour of capitation and patient lists.

I wonder if he has ever worked in a jurisdiction where such a system is in effect. I have worked in the Republic of Ireland, where this system has existed for many years after the shelving of fee per item.

It has proved to be a disaster for younger GPs. ( I hope they will be marching in the streets if such a system is ever mooted ...
