physicians organized to provide medical care to voluntarily
enrolled individuals for a fixed, prepaid, periodic payment.
The HMOs participating in the list underwrite risk for the
enrollees covered under their managed-care programs.

dependents for group health plans only.
(2) Group: A group model consists of hospitals and doctors
providing care and services exclusively for HMO recipients.
(3) Network: A network model is an HMO contracting with two or
more independent group practices (no solo practices) to provide
health care services.
(4) IPA: An individual practice association is the most common
type of arrangement for HMOs. Care is provided by a network of
independent physicians alone or in medical groups to care for
enrollees.
(5) Excludes physicians.
