United Cooperative Association Is Fined for Waukon, Iowa, Death.

Sep. 9--WAUKON, Iowa--The state has fined a Monona company in connection with the death of one of its workers in Waukon.
Donald Ralph Gray, 61, of Waukon, was found dead May 8 after he became trapped in a power take-off shaft that was part of his tractor trailer at the United Cooperative Association's farm fuel site in Waukon.
The Iowa Occupational Safety and Health Administration fined United Cooperative $2,500 for two violations related to not having guards around the spinning turbine that snagged and killed Gray, records state.
The company has since removed the old power take-off assembly and installed a hydraulic motor, records state.
