On Ford's E-150, E-250 and E-350 Super Duty vans, you can choose a sliding cargo door of one that opens out at no extra cost.
Shelves and storage compartments that can be accessed from outside the vehicle via drop-down, prop-up, lockable partitions are popular with those who need toolboxes at the ready. If the manufacturer of the van you want to buy doesn't offer the fittings you need, there are dozens of independent suppliers that can make the alterations.
