Medmarx version 5.0 is the latest edition of the national, anonymous medication error reporting program for hospitals and healthcare systems. The upgraded Internet-accessible program and quality improvement tool features: a redesigned interface; simplified integration; predefined reports; improved hospital comparisons, reporting and analysis; and enhanced charts and graphs to uncover patient patterns and trends. Users can: record important medication error information using standardized data formats, a nationally recognized severity index, drop-down menus, picklists and fields for recording causes and contributing factors; access a new primary reporting page that collects all mandatory fields into one screen for faster report entry to help hospitals avoid future errors by capturing information about actions taken and recommendations given; and draw on hospital system tools that facilitate comparisons, reporting and analysis across multiple sister facilities.
