Analyzing data collected on medical errors is an important part of improving patient safety; however, these analytical tools will not work if perioperative team members operate under a "code of silence" or fear retribution for speaking up. Health care facilities must develop a just culture--a work environment in which staff members are comfortable reporting errors or near misses when compliance with procedural requirements or safeguards is not met.
