The Chiari I Malformation & Syringomyelia: Case Studies--John J. Oro, MD

The Chiari Type I malformation is an uncommon, congenital, neuroskeletal deformity of the posterior compartment (fossa) of the cranial cavity and the associated neurological tissues. In the Chiari I malformation (CM-I), the cerebellar tonsils herniate into the upper spinal canal and crowd the outlet of the posterior fossa (foramen magnum) causing obstruction of spinal fluid flow. Though the cause is unknown, CM-I is thought to occur during early embryonic development and is more frequent in women than men. While CM-I has been noted from the first year of life to the eighth decade, it most commonly presents in adults during the third decade of life. Dysfunction of the brainstem, cranial nerves and cerebellum occurs, and abnormal spinal fluid flow can cause a cavity to form in the spinal cord (ie, syringomyelia). In view of the multiple neurological tissues involved, patients often present with a variety of neurological symptoms. We will review a number of cases to illustrate the complexities of these disorders.
