The fourth sentence should read: "If the medication is vancomycin ...

the infusion must be started 120 minutes before the anticipated time of initial incision to meet the recommendation that the infusion of the medication is complete within 60 minutes before the initial incision is made." The first sentence in the second paragraph should read: "... administration may be in the OR with the anesthesia care provider administering the medication, as long as it is infused within 60 minutes before the incision is made."
