(WX-unknown) DEATHS & INJURIES: 85 - Uninjured NTSB ID#: DCA00RA065 PRELIMINARY ANALYSIS2: A 28-inch crack in fuselage above forward service door between Sta. 300 and Sta. 328 at S-4R. Lap was repaired per Boeing Service Bulletin (SB)737-53A1039 in 1993. Preliminary research indicates some similarities to the catastrophic structural failure of Aloha Airlines' B-737-200 N73711, April 28, 1988 DATE & SITE: 6/13 - Peter-borough, Canada AIRCRAFT & REGISTRATION: Dassault Falcon 20 N184GA (Grand Air Express) CIRCUMSTANCES: Crashed short of runway after go-around from an abandoned NDB approach/IMC4/N5 DEATHS & INJURIES: 2 - Uninjured NTSB ID#: NYC00WA161 PRELIMINARY ANALYSIS2: The accident, from preliminary data, appears to fall into the broad category of CFIT (Controlled Flight Into Terrain). Investigation should determine if visibility minimums were appropriate for the approach. DATE & SITE: 7/1 -Strattan-ville, Pa. AIRCRAFT & REGISTRATION: Hartzell RANS-12 Not registered CIRCUMSTANCES: Crashed while maneuvering in traffic pattern/VMC3/N DEATHS & INJURIES: 2 - Fatal NTSB ID#: NYC00LA184 PRELIMINARY ANALYSIS2: Loss of control, entry into spin & inability to recover. Possible spatial disorientation during onset of darkness. DATE & SITE: 7/2 - Orlando, Fla. AIRCRAFT & REGISTRATION: Boeing 737-400 N460PR (Pro Air, Inc.) CIRCUMSTANCES: Airplane collided with cargo container loader while taxiing into gate/VMC/D DEATHS & INJURIES: 128 - Uninjured NTSB ID#: MIA00LA206 PRELIMINARY ANALYSIS2: Apparent failure of ground personnel to provide appropriate signals for taxiing airplane/possible failure of taxiing pilot to properly interpret, possibly non-standard, hand signals of ground crew DATE & SITE: 7/4 - Murfreesboro, Tenn. AIRCRAFT & REGISTRATION: Cessna 152, N89043 CIRCUMSTANCES: Loss of control resulting in crash on takeoff/ VMC/D DEATHS & INJURIES: 1 - Fatal 1 - Serious NTSB ID#: MIA00FA201 PRELIMINARY ANALYSIS2: Loss of control during takeoff/climb during apparent instructional flight/possible engine anomaly/actions or in-actions of Certified Flight Instructor (CFI) should be in question in investigation. AIRCRAFT & REGISTRATION: Piper PA-22 N1783P CIRCUMSTANCES: Crashed during approach/IMC/D DEATHS & INJURIES: 2 - Fatal NTSB ID#: CHI00FA184 PRELIMINARY ANALYSIS2: Apparently non-instrument qualified student pilot attempting flight in reported Instrument Meteorological Conditions (IMC); resulting in disorientation and loss of control. DATE & SITE: 7/6 - North Haven, Maine AIRCRAFT & REGISTRATION: Cessna U-206G N756HK (Telford Aviation, Inc.) CIRCUMSTANCES: Ran off departure end of runway during landing/VMC/D DEATHS & INJURIES: 3 - Uninjured NTSB ID#: NYC00LA193 PRELIMINARY ANALYSIS2: Failure of pilot to initiate and execute go- around/apparently excessive landing-touchdown speed, too far down runway and resulting inability to arrest landing roll within confines of runway. DATE & SITE: 7/7 - Rio Lujan,Argentina AIRCRAFT & REGISTRATION: Cessna 310N LV-JLB (Columbus Airways, S.A.) CIRCUMSTANCES: Crashed shortly after takeoff during reportedly adverse weather conditions/ IMC/D DEATHS & INJURIES: 5 - Fatal NTSB ID#: MIA00WA211 PRELIMINARY ANALYSIS2: Limited information precludes preliminary analysis.
