UB Law School Library authorized Passport Acceptance Facility

The University at Buffalo Law Library is now an authorized Passport Acceptance Facility. The certified passport agents in the law library are authorized to distribute forms, accept completed paperwork, review required documentation, and forward applications to the U.S. regional passport agency in Boston.

To obtain passport forms and general information, go to 211 O'Brian Hall (located inside the Law Library) at the University at Buffalo North Campus, Monday through Friday.

The following items are required for passport application:

* Two photos of passport quality;

* Citizenship documentation (Note: the library will send original documentation with the application. The documentation will be returned with your passport.)

* Photo ID (driver's license, military ID or other ID with good photo likeness);

* Check made out to Department of State or Passport Services ($55 application fee or $115 expedite fee); and

* Cash or check made out to UB Foundation ($30 execution fee).

Passport photos may be obtained on-campus at CVS (located in the Commons, North Campus).
