STATE OF OKLAHOMA TO: All persons, owners, producers, operators, purchasers and takers of oil and gas, and all other interested persons, particularly in Latimer County, Oklahoma, including the following: Amoco Production Company; Barbara D. Bergin; William R. Blair; Jo Ann Bruner; Chaparral Energy, Inc.; Gladys D. Cochran; Samuel E. Cochran, as Personal Representative of the Estate of James H. Cochran; Larry Lee Cole; Exxon Company USA; Exxon Corporation; Paula Cole Fly; Annette Henry; Cordell Henry; Cornelius D. Henry; Joseph Henry; Ross Henry; Sally Henry; Wayne Henry; Phyllis J. Marshall; Dorothy Mathews; June Laverne McFerran; Mobil Exploration & Producing US Inc.; Robert L. Moore; Dorothy L. Reed; Dorothy Willett Rogers; Karla Cole Starkey; The Trust Company of Oklahoma, as Special Administrator of the Estate of John Harmon McFerran; Union Oil Company of California; Boyd R. Willett; H.L. Willett; if any of the above-named individuals be deceased, then the unknown heirs, executors, administrators, devisees, trustees and assigns, both immediate and remote, of any such deceased individual; if any of the above-named entities is a dissolved partnership, corporation or other association, then the unknown successors, trustees and assigns, both immediate and remote, of any such dissolved partnership, corporation or other association; if the above named party designated as a personal representative is not presently acting in such capacity as personal representative, then the unknown successor or successors to such personal representative; and if the above-named party designated as a special administrator is not presently acting in such capacity as a special administrator, then the unknown successor or successors to such special administrator.
NOTICE IS HEREBY GIVEN that Applicant, Vastar Resources, Inc., has filed an application in this cause requesting the Corporation Commission of Oklahoma to enter an order, to be effective as of the date of the execution thereof or as of a date prior thereto, amending Order No. 395776 to authorize and permit an additional well, being the Blair Unit No. 5 Well, in the 640-acre drilling and spacing unit formed for the Fanshaw (Atoka) Sand common source of supply in Section 35, Township 7 North, Range 22 East of the IM, Latimer County, Oklahoma, so as to produce hydrocarbons from such common sources of supply, with such authorization and permission running in favor of Applicant or some other party recommended by Applicant, and to establish a proper allowable for such well and such unit.
NOTICE IS FURTHER GIVEN that the application in this cause requests that the order to be entered in this matter (amending Order No. 395776) be made effective as of the date of the execution thereof or as of a date prior thereto and that the authorization and permission requested herein run in favor of Applicant or some other party recommended by Applicant.
NOTICE IS FURTHER GIVEN that this cause is set before an administrative law judge for hearing, taking of evidence and reporting to the Commission.
NOTICE IS FURTHER GIVEN that this cause will be heard before an administrative law judge on the Conservation Docket at the Western Regional Service Office of the Corporation Commission, Jim Thorpe Building, 2101 North Lincoln, Oklahoma City, Oklahoma, at 8:30 A.M., on the 3rd day of June, 1996, and that this notice be published as required by law and the rules of the Commission.
NOTICE IS FURTHER GIVEN that the Applicant and interested parties may present testimony by telephone. The cost of telephonic communication shall be paid by the person or persons requesting its use. Interested parties who wish to participate by telephone shall contact Applicant or Applicant's attorney, prior to the hearing date, and provide their name and telephone number.
