Columbia Accident Investigation Board releases final report about

The Columbia Accident Investigation Board released its final report of the space shuttle Columbia disaster. It confirmed a chunk of foam broke off from the external fuel tank on liftoff and punched a hole in the left wing of the orbiter. The report found evidence of failure at Lockheed Martin Space Systems Michoud Operations, which builds the foam-covered fuel tanks for the National Aeronautics and Space Administration's shuttles.

The shuttle broke apart on re-entry into the earth's atmosphere on Feb. 1, killing all seven astronauts aboard.
